I have created my own formula for lip balm and have been considering offering personalization on the labels for events, conferences, parties, weddings, baby/bridal showers, etc.

Wondering if anyone offers their goods as promotional items, and if so, I'd love some feedback on the following:
1. how do you charge for the time involved to personalize a label?
2. Do you have set formats or allow them to custom order with graphics?
3. Do you require a finished design or do you work with them to create a custom label?
4. is there any kind of formula for how big your product name/logo is compared to their design/name?
5. where do you advertised your personalized product options?
